This form is for anyone who is organizing a "regular" Roman Catholic Mass for a parish, school or organization.
It is in English, and based on the official (ICEL) English translation of Novus Ordo, ie the Mass format introduced after Vatican II.
It can be adapted for special liturgies which have extra features: this has been done so far for weddings and funerals.

Fill in the background details that you want to include, eg:
The "music note" column shows how important it is to sing each item on the Mass, based on various church documents: You need to work with the celebrant decide which items are to be sung in this particular Mass, based on the situation and the resources and time available.
Choose appropriate Mass-parts and hymns, and write their names into the "item" column.
Enter the key that each song will be played in, and any notes about it (eg the introduction, number of verses, arrangement, soloist-only, etc)
If something on the form doesn't apply, then either leave it blank, or delete it.

Download a copy (right-click it, and choose "Save Link as"), or open it directly (click, and save or print the file that opens.Fill in the background details that you want to include, eg:
- Date and time
- Liturgical focus (usually based on the readings of the day and the feast or season)
- Pastoral focus (any local or personal events that are relevant to the people who will be attending, and which you want to acknowledge in your planning)
- The names of the priest(s), reader(s) and musicians and/or singers
